Digital skills survey – template

This survey is designed to help you assess the level of digital skills within your museum. Please customise this template to suit your particular museum/situation.

Introduction

Thank you for your participation in this survey, which will take approximately 10-15 minutes to complete.

Please note that in this survey the term ‘digital skills’ is used to refer to a broad range of knowledge and skills, which includes how to use digital tools, as well as ways of working and knowledge.

Your participation in this study is entirely voluntary and you can withdraw at any time. You are free to omit any question.

The information provided by you in this questionnaire will be used for research purposes only and all data will be stored securely at [insert details].
All data will be anonymised and will not be used in any manner allowing identification of individual responses. A summary of the results will be shared, but this report will not include any groups smaller than five.
OR [for smaller museums?]
We realise that it will be hard to answer questions anonymously, but your name need not be provided and will not be used if any data is published or shared beyond the museum/focus group. The more fully and honestly you are able to answer, the more useful your response will be in helping us work towards improving individual and organisational digital skills/literacy.
If you would like to discuss this survey or if you have questions, please contact [insert contact name, email address and/or phone number]
